By accessing and using the services of A+ Clinic, Lab and Pharmacy ("we," "our," or "us"), you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by the terms and conditions of this agreement, you are not authorized to use or access our services.

Pharmacy Services
  • Prescription medication dispensing
  • Over-the-counter medication sales
  • Medication counseling and consultation
  • Prescription refills and transfers
  • Specialized medication compounding

Patient Responsibilities

Appointment and Visit Responsibilities
  • Arrive on time for scheduled appointments
  • Provide accurate and complete medical information
  • Bring valid identification and insurance cards
  • Notify us of any changes to your contact information
  • Give at least 24 hours notice for appointment cancellations
Treatment Compliance
  • Follow prescribed treatment plans and medication instructions
  • Attend follow-up appointments as recommended
  • Inform us of any adverse reactions or complications
  • Report changes in your health condition promptly
Financial Responsibilities
  • Pay for services rendered in accordance with our fee schedule
  • Provide accurate insurance information
  • Pay applicable co-pays, deductibles, and non-covered services
  • Notify us of any billing discrepancies within 30 days

Appointment Policies

Scheduling
  • Appointments can be scheduled by phone, online, or in person
  • We will make reasonable efforts to accommodate your preferred time
  • Emergency appointments will be prioritized based on medical urgency
  • Walk-in patients will be seen based on availability
Cancellations and No-Shows
  • 24-hour advance notice is required for appointment cancellations
  • Late cancellations may result in a cancellation fee
  • Repeated no-shows may result in dismissal from the practice
  • Emergency situations will be considered on a case-by-case basis
Wait Times
  • We strive to see patients at their scheduled appointment time
  • Medical emergencies may cause delays in scheduled appointments
  • You will be notified of significant delays when possible
  • You may reschedule if wait times are unacceptable

Payment and Billing

Payment Methods
  • Cash, credit cards, debit cards, and bank transfers are accepted
  • Payment is due at the time of service unless other arrangements are made
  • Insurance co-pays and deductibles must be paid at the time of service
  • Payment plans may be available for qualifying patients
Insurance
  • We will verify your insurance coverage prior to service

Past Due Accounts

Pharmacy Policies

Prescription Dispensing
  • Valid prescriptions are required for all prescription medications
  • We verify the authenticity of all prescriptions
  • Controlled substances require valid identification
  • Generic substitutions may be made unless specified otherwise
Prescription Refills
  • Refill requests should be made 48-72 hours in advance
  • Valid prescriptions with remaining refills are required
  • New prescriptions may require physician authorization
  • Controlled substances have special refill requirements
Medication Returns
  • Prescription medications cannot be returned once dispensed
  • Over-the-counter items may be returned within 30 days
  • Returns require original receipt and unopened packaging
  • Defective products will be replaced at no charge

Limitation of Liability

Emergency Situations

Termination of Services

Patient-Initiated Termination

  • Outstanding balances remain due after termination
  • Medical records will be transferred upon written request
Provider-Initiated Termination
  • We may terminate services for non-payment or policy violations
  • 30 days written notice will be provided except in emergency situations
  • Emergency care will be provided during the notice period
  • Medical records will be made available for transfer

Changes to Terms

We reserve the right to modify these terms of service at any time. Changes will be:

  • Available for review at our facility
  • Communicated to patients when significant changes occur
  • Effective immediately upon posting unless otherwise specified

Your continued use of our services after changes are posted constitutes acceptance of the revised terms.
